Rock River Nursing & Rehab, a premier provider of healthcare services, is seeking a highly skilled and compassionate Registered Nurse (RN) - Per Diem to join our dynamic team. As a per diem RN, you will play a crucial role in delivering exceptional patient care while working a flexible schedule that meets your needs.

About the Role:

• Provide high-quality, individualized care to patients in a variety of settings, including skilled nursing facilities, assisted living facilities, and home healthcare.

• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans, promoting patient independence and well-being.

• Conduct patient assessments, develop treatment plans, and educate patients and families on disease management, medication, and self-care techniques.

• Document patient care accurately and thoroughly, ensuring compliance with regulatory requirements.

Responsibilities:

• Assess patients' physical, emotional, and psychosocial status to develop individualized care plans.

• Administer medications, treatments, and therapies as prescribed by physicians.

• Monitor patients' conditions, reporting changes and concerns to physicians and other healthcare professionals.

• Educate patients and families on disease management, self-care techniques, and prevention strategies.

• Participate in quality improvement initiatives, contributing to the development of best practices and policies.

Requirements:

• Current RN licensure in the state of Wisconsin .

• Basic Life Support (BLS) certification .

• Ability to work a flexible per diem schedule .

• Excellent communication, interpersonal, and problem-solving skills.

• Strong knowledge of nursing principles, practices, and procedures.
