We're seeking a compassionate and skilled Speech Language Pathologist (SLP) to join our team and make a meaningful difference in the lives of patients recovering from illness, injury, or managing chronic conditions. As a Home Health SLP, you'll evaluate, develop, and implement individualized treatment plans that improve communication, cognition, and swallowing while helping patients maintain their independence and quality of life. What You'll Do As a Speech Language Pathologist, you will: Evaluate patients with speech, language, cognitive, voice, and swallowing disorders. Develop personalized treatment plans based on comprehensive clinical assessments. Treat patients with dysphagia, aphasia, cognitive impairments, voice disorders, fluency disorders, and other communication challenges. Perform swallowing assessments and recommend therapeutic exercises, dietary modifications, and compensatory strategies. Educate patients and caregivers on communication techniques, home exercise programs, and safe swallowing practices. Recommend and train patients on alternative or augmentative communication systems when appropriate. Collaborate with physicians, nurses, therapists, and other interdisciplinary team members to ensure coordinated, high-quality patient care. Document evaluations, treatment plans, patient progress, and outcomes accurately and in compliance with agency and regulatory standards. Participate in case conferences and interdisciplinary meetings to support exceptional patient outcomes. Qualifications Required: Current Speech Language Pathologist (SLP) license in the applicable state Current CPR certification Valid driver's license Reliable transportation with current auto insurance Preferred: Minimum of one year of rehabilitation or speech therapy experience Home Health experience Long-Term Care experience HomeCare HomeBase (HCHB) experience Why Join Choice Health at Home?
