Key Responsibilities

• Performs high-level triage of all patients within 24 hours of admission, with focus on identifying complex psychosocial, financial, placement, and community service needs.
• Assesses barriers early in the patient's stay and formulates a discharge plan in collaboration with the patient, family, physician, payers, and the broader healthcare team.
• Evaluates risk of readmission for specified patient populations and initiates appropriate interventions to support successful transitions along the care continuum.
• Identifies, arranges, and participates in family care conferences and interdisciplinary rounds; provides consultation to patients, families, and clinical staff.
• Manages complex cases and advocates for patients and families throughout the development of the care and discharge plan, including coordination with external agencies and post-acute facilities.
• Documents assessments, plans, interventions, barriers, and reassessments in the EMR to facilitate safe and timely transitions to the next level of care.

• Licensed Master's Social Worker (LMSW) - LMSW certification required within one year of eligibility; ongoing continuing education requirements must be maintained.
• Licensed Bachelor Social Worker (LBSW) - State licensure required; LBSW certification must be obtained within one year of eligibility; ongoing continuing education requirements must be maintained.
• Acute Care experience required.
• Active Michigan state license required.
• Candidates residing within 35 miles of the facility are considered local travelers and may be subject to an adjusted rate.

• Minimum three years of acute hospital care or social work experience.
• Basic Life Support (BLS) certification as a Healthcare Provider through the American Heart Association, American Red Cross, or equivalent Military Training Network (MTN).
• Completed skills checklist within one year of application.
