Kidney Care Navigator — Virtual & Field Outreach

Fresenius Medical Care

Fresenius Medical Care is seeking a Kidney Care Specialist to support patients with Chronic Kidney Disease and End Stage Renal Disease. This role involves virtual care navigation, patient education, and collaboration with nephrologists and care teams to ensure effective patient support. Preferred candidates will have a High School diploma and 1-3 years of experience in kidney care management. Knowledge in telephonic patient education and strong communication skills are essential for engaging patients effectively.
