Social Service Manager - Lake County, IL What you will do What You’ll Do Must be able to travel throughout northern Lake County Caregiver Assessments using the T‑CARE model Information & Referral for seniors and caregivers Case Documentation in My Senior Center and agency systems Facilitate Dementia Support Groups Launch a Memory Café in the region Support reporting and Title IIIE Caregiver Resource activities What We’re Looking For Experience working with caregivers Strong communication and interpersonal skills Ability to work independently and problem‑solve Attention to detail and strong documentation habits Must be able to travel throughout northern Lake County #J-18808-Ljbffr Bacoa
