Our growing home health agency is seeking a compassionate and experienced full-time RN Case Manager to join our team. Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient's physician orders and plan of care under the direction and supervision of the RN Clinical Manager and Administrator.
