Physical Therapist

We are looking for a part-time or full-time Physical Therapist to join our growing rehabilitation clinic. This position is based at our outpatient clinic in Deerfield, IL. Responsibilities: Evaluate patient and gathers appropriate information to determine accurate clinical diagnoses and treatment plan. Establish appropriate, functional and objective goals based upon clinical examination. Provide exceptional patient care and service. The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive and respectful working relationships. Cultivate relationships with patients, doctors and team members. Provide continuous communication with referring physicians and other referral sources to ensure a consistent level of care. Maintain patient charts, clinical logs and checklists necessary for compliance. Maintain patient confidentiality with regard to medical records per company policies and procedures and assures compliance with all HIPAA guidelines. Education and/or Experience: Bachelor's, Masters or Doctorate Degree from an accredited Physical Therapy Program. Current State of Illinois Physical Therapy license.
