About the Job: Assist provider as necessary; room patients, take vitals, triage patients, call in scripts, and other associated responsibilities.

What You'll Do:

• Carry out the nursing process in person, by telephone (telephone triage), and via telehealth
• Participate in designing and implementing a plan of care with the interdisciplinary team including Registered Nurses, Physicians, Physician Assistants, and other health professionals.
• Participate in educating patients on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).
• Administers prescribed medications, treatments, and vaccines, performing point-of-care testing

Who You Are:

• Graduate from an accredited school of nursing required.
• Current NH Registered Nursing (RN) or Licensed Practical Nursing (LPN) licensure required.
• Prior work experience in a healthcare setting required.
• Current BLS/CPR training.

Work Shift: 36 Hours | Day Shift | Monday - Friday 8am - 5pm
