Job Description & Requirements Specialty: Hospice Discipline: RN Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Hospice Registered Nurse (RN) / Case Manager Shift: Days Schedule: 8-hour shifts, 40 hours/week Main responsibilities Coordinate comprehensive end-of-life care for terminally ill patients. Manage an assigned hospice caseload. Perform patient assessments and create individualized care plans. Provide skilled nursing care and symptom management. Administer medications and monitor effectiveness. Address patients' physical, emotional, and psychosocial needs. Educate patients and families about care and symptom management. Coordinate care with physicians and the interdisciplinary hospice team. Maintain communication and continuity of care. Continuously evaluate and adjust the plan of care. Required: CA RN License BLS 5 years of Hospise and Case Management Experience Reliable car Auto insurance.
