Registered Nurse Case Manager

Job Overview Functional Title: Registered Nurse Case Manager Job Title: Nurse III Agency: Health & Human Services Commission Department: Cottages - Nursing Posting Number: 18615 Salary Range: $7,532.75 - $8,886.16 per month Salary Group: TEXAS-B-24 Shift: Day (minimum 5% travel, no telework) Facility Location: Brenham State Supported Living Center, 4001 S HWY 36, BRENHAM, TX Key Responsibilities The RN Case Manager performs complex nursing work providing assessment, care and integrated treatment for residents at the State Supported Living Center (SSLC). Responsibilities include: Oversee the delivery of nursing care and monitor health status to meet individual needs. Work closely as part of an interdisciplinary team, ensuring communication and holistic care. Perform physical assessments, document needs, develop health care plans, and supervise implementation of nursing interventions. Conduct rounds on assigned homes, gather information, and observe residents. Mentor and coach new RN case managers. Participate in quality assurance reviews, committees, and workgroups. Attend medical appointments and coordinate care with providers. Perform trend analysis of health care needs and report findings. Maintain competence in nursing best practices and regulatory knowledge. Essential Job Functions Attend work on a regular basis, including potential rotating schedule or overtime. Serve in on‑call status as required. Monitor health status of all assigned individuals and conduct rounds to gather information. Oversee nursing interventions and evaluate outcomes. Document and follow up on issues per protocols. Provide hands‑on nursing care and guidance to LVNs and staff. Participate in interdisciplinary team meetings and problem‑solving. Educate team members, LARs, and individuals about care plans. Update system records and communicate information. Perform physical assessments on a monthly, quarterly, and annual basis. Complete required documentation on schedule (daily, weekly, quarterly, annual). Review physician orders and Medication Administration Records. Attend on‑campus medical appointments and coordinate off‑campus care. Engage in competency training and continuing education. Maintain safety, infection control, and information security protocols. Use ability to work independently and collaborate with all staff levels. Support agency disaster response and COOP activation as needed. Knowledge, Skills & Abilities Knowledge of nursing techniques, standards, and procedures; health care laws and regulations. Skill in patient care, assessment, documentation, and interpretation of policies. Knowledge of state and federal laws governing ICF‑IID facilities. Ability to interact with residents, families, and staff. Skill in computer, electronic health records, time management and prioritization. Excellent verbal and written communication skills. Ability to work independently and with all staff levels. Current CPR certification and physical ability for CPR. Knowledge of emergency procedures and equipment. Ability to interpret regulations and communicate them effectively. Ability to perform comprehensive physical assessments and recognize abnormal findings. Ability to analyze information and make nursing judgments. Licenses & Certifications Licensed to practice as a registered nurse in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact. Initial Screening Criteria At least one year experience as a practicing registered nurse. Experience in assessment and documentation of health status. Preferred: at least two years experience in a health agency, hospital, or health‑care facility. Preferred: Bachelor's Degree. Preferred: Experience working with individuals with intellectual or developmental disabilities (IDD). Salary & Benefits Starting salary: $7,532.75 per month.
