Occupational Therapist (OT) – Skilled Nursing Facility (SNF) 60;

We are 60;seeking 60;a skilled and compassionate 60;Occupational Therapist (OT) 60;to join a collaborative therapy team within a skilled nursing facility setting. This position focuses on providing high-quality rehabilitation services to a primarily geriatric population, including short-term rehab patients and long-term care residents. 60;

The OT will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions designed to restore functional independence, improve safety, and enhance overall quality of life. This role requires strong clinical judgment, efficiency, and the ability to thrive in a fast-paced, productivity-driven environment. 60;

Key Responsibilities: 60;

• Perform comprehensive occupational therapy evaluations and 60;establish 60;individualized plans of care 60;

• Provide skilled treatment targeting ADLs, functional mobility, strength, balance, cognition, and safety awareness 60;

• Implement fall prevention strategies and environmental modifications 60;

• Recommend and train residents in adaptive equipment and compensatory techniques 60;

• Complete 60;accurate 60;and 60;timely 60;documentation, including evaluations, progress notes, recerts, and discharge summaries 60;

• Collaborate with interdisciplinary team members including PT, SLP, nursing, and case management 60;

• Supervise and direct COTAs 60;in accordance with 60;state practice guidelines 60;

• Maintain productivity standards while ensuring patient-centered care 60;

Ideal Candidate: 60;

• Active OT license in practicing state 60;

• SNF or geriatric experience preferred 60;

• Strong clinical reasoning and time management skills 60;

• Comfortable managing both short-term rehab and long-term care caseloads 60;
