Schedule: Monday through Friday day schedule with rotating on-call responsibilities

Requirements:

• Active Registered Nurse (RN) license in the State of Colorado or active NLC compact license
• Valid driver's license, reliable transportation, and current auto insurance
• Strong assessment and critical thinking skills with the ability to work independently
• Clear, professional communication skills for interaction with patients, families, and healthcare team members
• Proficiency with clinical documentation and EMR systems
• Home health experience preferred but not required

Position Overview:

This is a mobile, field-based clinical position where you will travel throughout the service area to conduct scheduled home health visits. Responsibilities include performing comprehensive nursing assessments, delivering skilled nursing interventions, developing and updating individualized care plans, and educating patients and families on disease management, medications, and treatment protocols. You will monitor patient progress during home visits, communicate changes in condition to providers and the care team, and complete thorough documentation in the EMR for each visit. This is not a remote, telephonic, or traditional case management role—it is hands-on, direct patient care delivered in the home setting.
