Physical Therapist AssistantThe Physical Therapist Assistant provides outpatient rehabilitation services in an in-home setting for the older adult population. Treatment settings include private residences and independent living facilities. This role allows treatment in the patient's most functional environment—the home. Under the supervision of a Licensed Physical Therapist, the Physical Therapist Assistant delivers physical therapy interventions to promote improved functional mobility, independence, and safety. The position offers a flexible schedule and work environment across various settings.Responsibilities / AccountabilitiesParticipate in patient care conferences, staffing meetings, and required meetings under the direction of the Physical TherapistAdhere to departmental schedules and complete timely clinical and billing documentationCollaborate with interdisciplinary rehabilitation team members to implement patient-centered treatment programsAssist the Physical Therapist during evaluations, testing, and complex proceduresEducate patients, families, and caregivers on physical therapy techniques and home programsMaintain accurate, complete, and timely documentation for all patients treatedClean, maintain, and report repair needs for therapy equipmentDeliver patient-centered care with compassion and respect for individual needs and rightsImplement individualized treatment plans established by the supervising Physical TherapistProvide individualized physical therapy interventions, including but not limited to:Therapeutic exercise (strength, endurance, balance, coordination, flexibility, gait, posture, neuromotor development)Functional trainingManual therapyAirway clearance techniquesIntegumentary repair and protectionElectrotherapeutic, physical agent, and mechanical modalitiesFalls risk interventionsHealth and wellness programsChronic disease management educationPerform other related duties as assignedEducational / Vocational RequirementsValid Physical Therapist Assistant license in the state(s) of practice or proof of license eligibilityValid driver's license and reliable transportation to patient residencesJob SkillsStrong organizational and time management skillsEffective verbal and written communication skillsComplianceComply with all applicable laws, regulations, policies, and ethical standards, including HIPAAParticipate in required orientation and training programsPromptly report suspected non-compliance through appropriate reporting channelsCooperate with audits, monitoring, and investigationsParticipate in quality assurance and process improvement activities as requestedEssential Job FunctionsPhysical DemandsRequired Abilities:Reaching (overhead), handling, fingering, feeling, fine manipulationTalking, hearing, and visual acuityFrequency:Standing: Frequently (67–100%)Walking: Frequently (67–100%)Sitting: Occasionally (34–66%)Lifting / Carrying / Pushing / Pulling:Up to 50 poundsPostural Activities:Climbing stairs, balancing, stooping, kneeling, crouching, crawling, twistingWork EnvironmentInside: 90% | Outside: 10%Exposure to noise, vibrations, hazards, fumes, uneven ground, and moving machineryOperation of automotive equipmentOccasional use of protective equipment (gowns, gloves, masks)Work SituationsPerformance measured by objective criteriaFrequent interaction with patients and caregiversRepetitive tasks and work under time or physical demandsProblem solving and ability to follow instructions
