Job Description
Title: Occupational Therapist

Schedule: 8 hours

Assignment Details: Contract Length: 13 weeks, Guaranteed Hours: 40

Requirements:
- Current state license as an Occupational Therapist
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
- Two years of experience as an Occupational Therapist in an acute care or rehabilitation setting
- Graduate of an accredited school of occupational therapy with a Bachelor of Science degree in Occupational Therapy

Roles & Responsibilities:
- Provide coordinated care and individualized patient care plans in a home care environment.
- Assess and facilitate patients' functional mobility, safety issues, and the use of adaptive equipment.
- Evaluate patient occupational performance in self-care, work, leisure, and rest.
- Document clinical assessments, progress notes, and maintain accurate records.
- Collaborate with interdisciplinary teams and communicate relevant patient information while ensuring confidentiality.
- Promote compliance with third-party payers through education and support.
