Nurse Practitioner (NP) OpportunityLake Sunapee VNA and Hospice is seeking an experienced Nurse Practitioner (NP) to join our Hospice program in a per diem capacity to provide direct patient care visits for routine provider visits, symptom management and Hospice Face to Face. On-call rotation required.Lake Sunapee Region VNA & Hospice provides home health care and hospice services that help individuals remain safe, healthy, and independent in the comfort of their own homes throughout Grafton, Merrimack, and Sullivan counties in New Hampshire.You're a Nurse Practitioner (NP) who values personalized, patient-centered care and meaningful connections with patients and families.
