Tripod Partners is seeking an experienced RN Care Manager for a Home Health role (non-hospital setting) in Marysville, CA. This position involves coordinating patient care, conducting in-home visits, and managing care plans across a diverse patient population.

Candidates must have at least 2 years of nursing experience, including recent Home Health experience, along with a background in care management. An active RN license in the state of practice and AHA BLS (or equivalent) certification are also required.

A BSN and case management certification are preferred.
