This role is essential in providing high-quality patient care in a dynamic environment, whether in a teaching hospital or urgent care setting. The ideal candidate will possess strong clinical skills and a commitment to patient education and advocacy. As a UAS Assessor, you will conduct in-person assessments for clients to evaluate their eligibility and ongoing needs, contributing to crucial care and support plans./ppResponsibilities/pp Conduct UAS assessments for clients to determine their level of care and support needs./pp Utilize the Uniform Assessment System (UAS) tool to document assessment results accurately./pp Provide compassionate and professional interactions with clients in various community settings./pp Submit completed assessments within designated timeframes and maintain detailed records./pp Collaborate with other care providers to ensure a comprehensive assessment process./pp Attend required training sessions and stay informed of assessment updates./ppQualifications/pulliRegistered Nurse, currently licensed and registered to practice in New York State./liliUAS-NY Certified/liliBachelors degree in nursing preferred./liliExperience in Home Care, case management, or long-term home care./liliDemonstrated skills in planning, organizing and directing the work of others required./liliExperience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments./liliProficient with DOH UAS assessment instrument./li/ulpExperience/pulliUAS-NY Certified/li/ulpLicense/Certification/pulliBLS Certification (Preferred)/liliRN License (Required)/li/ulpJob Type: Fee for Service, Per diem/ppPay: $140.00 per visit-$165.00 per visit/ppWe are excited to welcome passionate and dedicated individuals to join our team at Personal Touch Home Care.
