Certified Occupational Therapy Assistant (COTA) - Full-Time or Part-Time

Location: Lancaster & Mineral Point, WI
Facilities: Lancaster Health Services and Mineral Point Health Services
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team serving Lancaster Health Services in Lancaster, Wisconsin, and Mineral Point Health Services in Mineral Point, Wisconsin .

This Full-Time or Part-Time Occupational Therapy Assistant opportunity offers the chance to provide meaningful patient care in supportive, collaborative skilled nursing facilities where your clinical expertise makes a daily impact.

As a COTA , you will work alongside a licensed Occupational Therapist to provide skilled occupational therapy treatments that help adult and geriatric patients improve independence, safety, mobility, and quality of life.

• Provide occupational therapy treatments under the direction and supervision of a licensed Occupational Therapist.
• Implement individualized treatment plans designed to improve patients’ functional abilities and independence.
• Assist patients with activities of daily living (ADLs), mobility, coordination, strength, cognition, and safety awareness.
• Monitor patient progress and communicate updates with the supervising Occupational Therapist.
• Collaborate with Occupational Therapists, nursing staff, physicians, caregivers, and interdisciplinary team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety strategies.
• Provide compassionate, patient-centered occupational therapy services.

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Wisconsin Certified Occupational Therapy Assistant (COTA) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
