Mental Health Consultant

Immediate opening for a Full-Time Mental Health Facilitator in our Willow Grove location for our Bucks County High Fidelity Wraparound Family Teams Program. The High-Fidelity Wraparound process; known in Bucks County as High Fidelity Family Teams, is a needs based planning process to improve the lives of children with complex mental health needs and their families by empowering the families and youth to have their voices heard through an individualized planning process. The individualized plan is child-centered and family-focused with the process focusing on strengthening the natural family, extended family and social supports for the child and family by developing a team to support the family throughout the process and in learning the skills of the model. Responsibilities Collaboratewith the youth and their family throughout Bucks County to engage them in the High Fidelity Family Teams process and help them to identify and express their needs. Collaboratewith otherservices providers and natural support involved with the youth and family while managing the team process. Ensurethat the team mission, goals, and plan address the youth and family’s needs. Monitorimplementation of the plan and communicatefrequently with the team to identify any emerging needs that mayneed to be added to the plan. Teach and transfer theHigh Fidelity skillsto the youth, family and natural supportsby modeling, building and monitoring the youth and family’s self-efficacy. Workwith the team assisting the youth and family in identifying and strengthening their natural and social support system. Monitorsatisfaction of all team members while strengthening and maintaining engagement. Use and implement the 5 core values of Trauma-Informed Care in all work practices - Safety, Trustworthiness, Choice, Collaboration, & Empowerment.
