Identify and address the psychosocial needs of residents and their famil Perform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors Formulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning.Include family in teaching as appropriate. Demonstrate knowledge of clinical social work and discharge planning.Interpret and promote residents’ rights and the “Residents’ Bill of Rights” Interact professionally with resident and family and involve resident and family in the formation of the plan of care. Provide individual and group help for residents’ and families at times of adjustment, crisis, or need. Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met and facility policy is followed. Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff. Be flexible, organized, and function under stressful situations. Maintain a good rapport and working relationship with medical staff, departments, and employees. Manage and operate equipment safely and correctly. Treat patients and their families with respect and dignity. Support and maintain a culture of safety and quality. Qualifications of Social Services Director: Bachelor’s degree in social work or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology One year of supervised social work experience in a health care setting working directly with individuals is preferred Current State Clinical Social Work Licensure Candidates must maintain a working email address and phone number for employer communication. Ability to work cooperatively as a member of a team Ability to communicate effectively with residents and their family members, and at all levels of the organization Ability to react decisively and quickly in emergency situations Ability to maintain confidentiality Ability to read, write and speak the English language Knowledge of training techniques for clinical staff Good Ethical Practice Relationship management Our facility provides equal employment opportunities.
