Job Requirements Bachelor’s (or higher) degree in Occupational Therapy from an accredited program. Successful completion of the National Certification Examination for OTR. Current state licensure or license eligibility. Assumes responsibility for ongoing continuing education and professional development. Job Responsibilities Provides and directs occupational therapy services, including assessment, treatment, program planning and implementation, related documentation, and communication under physician orders. Initiates referrals as appropriate in accordance with policy. Conducts patient screenings at regular intervals to determine intervention needs. Performs evaluations to obtain data for treatment planning and conducts specialized evaluations as indicated. Interprets and communicates evaluation findings to patients, families, and team members. Establishes individual patient schedules with other team members. Develops treatment plans with short and long‑term goals and methods to achieve them. Coordinates treatment plans with patients, families, and team. Implements and conducts treatments as outlined. Monitors response to intervention and modifies treatment to attain goals per physician orders. Develops appropriate home or community programs to maintain patient performance. Documents assessment, treatment, follow‑up, and termination results. Adapts treatment to meet individual needs. Provides, recommends, or fabricates adaptive devices; trains patients, families, and staff in orthotics and prosthetics. Participates in discharge planning. Supervises and directs COTAs, rehabilitation techs/aides, students, and volunteers. Reviews quality and appropriateness of services and therapy programs for effectiveness and efficiency. Participates in patient, family, and staff education. Confers with team members to coordinate total rehabilitation program. Adheres to documentation procedures for occupational therapy services. Attends in‑service and facility meetings as required. Provides in‑service education to the care team and community. Delivers treatment in accordance with standards of practice and departmental procedures. Maintains a safe, clean work area and complies with security, infection control, and OSHA standards. Demonstrates sound judgment in evaluation, planning, implementation, and follow‑up. Maintains positive relationships with coworkers, supervisors, patients, families, and administration. Reports to work on time and coordinates schedules for efficient treatment. Assists with and participates in the quality assurance program as needed. Adheres to confidentiality standards. Projects a professional image at all times. May travel for home assessments within reasonable distance from facility. Maintains passwords for required software securely. Performs other duties as assigned to effectively fulfil responsibilities and serve the best interest of Northwoods Rehab.
