Internal Medicine PhysicianLifekind Health is looking for full-time Internal Medicine Physicians to join our team. Our team of medical doctors, psychologists, chiropractors, acupuncturists, and dietitians work together within a revolutionary transdisciplinary model that addresses the quadruple aim of healthcare: enhancing patient experience, improving patient health, reducing healthcare costs, and increasing employee satisfaction.Schedule: Monday – Friday, 8:00 am – 5:00 pm with rotating weekends; on-call weekendsLocation: Rancho Mirage, San Jacinto, or Victorville, CA - must be open to float to any of our 8 clinicsMedical Evaluation and Management:Conduct comprehensive patient evaluations including history, physical examination, and medical decision-makingDiagnose and manage acute and chronic medical conditionsDevelop and implement individualized, evidence-based treatment plansMonitor patient progress and adjust care plans as clinically indicatedChronic Disease Management, CCM, and Risk Stratification:Provide longitudinal management of patients with chronic conditions (e.g., hypertension, diabetes, obesity, chronic pain, behavioral health comorbidities)Oversee and support Chronic Care Management (CCM) services, ensuring:Appropriate patient identification and enrollmentDevelopment and maintenance of comprehensive care plansOngoing clinical oversight of non-face-to-face care coordination servicesPerform risk stratification and implement mitigation strategies for high-risk patientsEnsure appropriate monitoring, including laboratory testing, urine drug screening, and functional assessmentsValue-Based Care (VBC) and Population Health:Participate in Value-Based Care initiatives focused on:Improving quality metrics and patient outcomesReducing emergency department visits and hospitalizationsEnhancing patient engagement and adherence to care plansCollaborate with Population Health teams (RN, LVN, Care Coordinators, CHWs) to address:Social determinants of healthCare gaps and preventative servicesSupport accurate attribution, documentation, and reporting of quality measuresCare Coordination and Transdisciplinary Integration:Collaborate with transdisciplinary team members including: Advanced Practice Providers (APPs), Behavioral Health, Physical Reconditioning, Dietitians, Population Health staffParticipate in structured team communication processes (e.g., SCRUM, case reviews)Ensure coordinated, patient-centered care across all servicesInterventional and Specialty Care Support:Evaluate and medically optimize patients for interventional procedures and advanced therapiesProvide medical clearance and risk assessment when indicatedCoordinate with specialty providers to ensure appropriate patient selection and continuity of careAPP Supervision and Collaboration (California Compliance):Provide supervision and/or collaboration with Nurse Practitioners (NPs) and Physician Assistants (PAs) in accordance with California Business and Professions Code and applicable regulationsMaintain and adhere to written supervisory or collaborative agreements as requiredEnsure appropriate availability for consultation, including in-person, telephonic, or electronic communicationConduct periodic chart review, case discussion, and quality oversight consistent with organizational policy and state requirementsSupport APP clinical decision-making, particularly in:Complex or high-risk patientsControlled substance prescribingInterventional or advanced therapy referralsDelegate services to APPs consistent with their scope of practice, training, and competencyParticipate in onboarding, training, and ongoing competency validation of APPsDocumentation and Compliance:Complete accurate and timely documentation for all encounters and CCM servicesEnsure documentation supports:Medical necessityCPT/ICD-10 coding accuracyVBC quality reporting requirementsFinalize and sign records within established organizational timelinesMaintain compliance with CMS, payer, and regulatory requirementsQuality, Outcomes, and Performance:Participate in quality improvement and performance initiatives aligned with VBC modelsUtilize clinical data, PROMs, and outcome measures to guide care decisionsSupport organizational goals related to:Patient outcomesCare plan adherenceReduction in patient disengagement/disenrollmentCollaboration and Clinical Oversight:Provide clinical collaboration and support to Advanced Practice ProvidersParticipate in clinical meetings, case reviews, and organizational initiativesMaintain professional communication and leadership within the care teamOur Ideal Candidate will have the following qualifications:Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) requiredCompletion of an accredited residency program in Internal Medicine or Family MedicineActive, unrestricted medical license in the state of CaliforniaBoard Certified or Board Eligible in Internal Medicine or Family MedicineActive DEA registrationKnowledge of chronic disease management and population health principlesUnderstanding of Chronic Care Management (CCM) and Value-Based Care (VBC) modelsAbility to manage complex and high-risk patient populationsStrong clinical judgment and decision-making skillsEffective communication and collaboration within team-based care modelsProficiency in electronic health record (EHR) systemsAt Lifekind, we believe our employees deserve the best.
