Licensed Practical Nurse (LPN) - Per Diem Home Health - Jupiter, FL

Matrix Home Care is seeking experienced and compassionate Licensed Practical Nurses (LPN) to perform home health visits in the Vero/Sebastian, FL area. This role is critical in initiating patient care, completing comprehensive assessments, and ensuring accurate OASIS documentation for home health patients.

• Provide home-based care to patients with illnesses, injuries, and chronic conditions, often following discharge from a hospital, rehabilitation center, or nursing facility
• Examine patients, complete clinical assessments, document progress, and report changes to physicians
• Complete accurate and timely clinical documentation , including OASIS, evaluations, physician orders, and care coordination notes
• Develop, implement, and continuously monitor patient care plans in collaboration with patients and their representatives
• Deliver skilled nursing services and provide case management for assigned patients
• Educate patients on plan of care, medications, therapies, diet, and exercise
• Coach and support family members on patient care, medication management, and home safety
• Coordinate care with physicians, discharge planners, and interdisciplinary teams
• Oversee and collaborate with LPNs, CNAs, and home health aides providing patient care
• Monitor patient progress, outcomes, and changing needs, adjusting care plans accordingly
• Ensure care goals are met and coordinate appropriate discharge from services
• Serve as a liaison with community health resources, including DME providers, oxygen services, and infusion therapy providers
• Maintain ongoing communication with patients, families, physicians, clinical managers, and office staff
• Participate in interdisciplinary team meetings and staff conferences
• Engage in continuous education and professional development opportunities
• Develop and follow work methods and procedures that support high-quality patient care

Qualifications

• Active Florida Licensed Practical Nurse (LPN) in good standing
• Minimum 1 year of home health experience required
• Strong experience with OASIS documentation
• Experience with KanTime EMR system preferred
• Excellent clinical assessment and critical thinking skills
• Ability to work independently and manage a flexible schedule
• Reliable transportation and valid driver's license
