Seven Healthcare are seeking a qualified and dedicated Physical Therapist to join their team in Valdosta, GA on a contract-to-hire basis. Emphasizing a travel assignment type, this role requires a professional skilled in home health settings. You will be working across Lowndes, Echols, Brooks, Cook, Berrien, and Lanier counties, utilizing your expertise to enhance the quality of care provided to patients in need.

- Perform physical therapy evaluations and treatments based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnosis, history, precautions, and contraindications
- Establish measurable goals and develop treatment plans
- Regularly reassess treatment plan effectiveness and patient progress
- Communicate patient needs and progress with the treatment team and family
- Consult with medical providers regarding evaluations, treatments, and discharge
- Recommend and assist in procuring adaptive equipment for patients
- Maintain and submit documentation as required by regulations
- Supervise Physical Therapist Assistants and Aides as specified by state practice act
- Participate in staff development programs and obtain continuing education
- PT license in GA or compact licensee accepted
- Experience with HomeCare Homebase and home health (2 years)
- OASIS certification
