Nurse Practitioner, Advanced Practice Provider

Position Overview Primary Care Physician – Oak Street Health, located at 11600 Farm to Market 1960 Rd W, Houston, TX 77065. Full‑time, Monday through Friday, 8:00 AM – 5:00 PM. The role involves delivering equitable, value‑based primary care to Medicare patients within a neighborhood primary care network. Responsibilities Assess and diagnose patients at local clinics. Oversee, direct, and administer primary care services. Prescribe and administer pharmaceutical treatments and medication. Maintain patient electronic medical record data via Canopy and Greenway systems. Collaborate with regional and central leadership to meet health quality goals. Work with Practice Managers to direct and manage the center care team. Perform other duties as assigned. Required Qualifications Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate in Internal Medicine or Family Medicine, with board certification (or board eligible) Active, non‑probationary, unrestricted State License Active DEA license U.S. work authorization Embodies the “Oaky” mindset (“radiating positive energy,” “assuring good intentions,” etc.) Preferred Qualifications Fellowship training in Geriatrics Experience practicing medicine with geriatric populations Experience operating in a value‑based healthcare model Experience in a collaborative setting that ensures positive health outcomes Experience in outpatient primary care settings Bilingual proficiency where applicable Benefits Competitive salary with quarterly bonus based on quality metrics.
