Licensed Practical Nurse (LPN) - Concord Care Center of Hartford

Responsibilities:

• Provide direct resident care under the supervision of an RN or physician
• Administer medications and treatments accurately and safely
• Monitor and document resident progress and condition changes
• Communicate effectively with residents, families, and interdisciplinary staff
• Promote a warm, safe, and respectful environment
• Assist in maintaining compliance with state and federal regulations

Qualifications:

• Active LPN license in the State of Ohio (required)
• Skilled nursing or long-term care experience preferred
• Excellent teamwork and communication skills
• Compassionate and patient-focused approach
