divh2Medical Coordinator (LVN)/h2pThe Medical Coordinator (LVN) is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights./ppThe pay range for this position is $34.00 - 36.00 per hour, depending on experience. This position also qualifies for $1.50 swing shift differential and $3.00 graveyard shift differential. The schedule for this position is 11:00pm to 7:30am, 5 days on, 2 days off. Work site may vary based on operational needs./ph3Responsibilities/h3pClient Care:/pulliPerform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes/liliAssess health status of patient upon arrival by conducting head to toe assessment and checking vitals/liliDetermine the patients insurance status, make follow up appointments with patients providers and reviewing medication list/liliEstablish and maintain a care plan/liliSet weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs./liliConduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge./liliLink clients to medical services when necessary (insurance, PCP, specialists, referrals)./liliCoordinate home health visits (if needed) and follow-up on medical appointments./liliPromote health literacy by providing patient education with regards to clients illnesses, medications, preventative measures, treatment management and overall general health./liliArrange transportation for clients to and from medical appointments, as needed/liliReport emergency situations to Supervisor/Manager and document incident/liliSeek proper level of care to assess and treat medical complication/liliInitiate medical response system when appropriate/liliSupport patient recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility./li/ulpAdministrative/Record Keeping:/pulliEnter patient information and case notes into EHR and other documentation systems./liliComplete and update medication, vitals, progress notes and care plan/liliPerform administrative tasks such as answering phones and filing paperwork./liliPrepare patient reports for billing./liliMaintain proper inventory of medical supplies and medications./li/ulpCompliance/Safety:/pulliMaintain confidentiality in accordance with HIPAA guidelines, Illumination Foundation policies and DHS standards./liliProvide patients with proper compliance forms upon admission/liliUtilize infection control measures according to OSHA standards./liliConduct medication reconciliation and ensure proper disposal and pickup of bio-medical/pharmaceutical waste./liliMaintain a safe work environment by ensuring the cleanliness of the med room, wound care room, and medication storage room./li/ulh3Qualifications/h3pRequired:/pulliCurrent and non-restricted LVN license in California/lili2 years of relevant experience./liliHigh School Diploma or equivalent./liliBasic computer skills, including the ability to send and receive emails and summarize data in spreadsheets./liliValid CA Drivers License./li/ulpPreferred:/pulliAvailability to work weekends and nights/liliHome Health nursing experience and experience working with populations dealing with addiction and behavioral health conditions./liliBilingual in English and Spanish./liliProficiency in Microsoft Office Suite (Outlook, Word, Excel, Calendar) and KIPU or other electronic health record systems./li/ulh3Benefits/h3ulliMedical Insurance funded by Illumination Health + Home (Kaiser and Blue Shield), depending on the plan/liliDental and Vision Insurance/liliLife, ADD and LTD Insurance funded 100% by Illumination Health + Home/liliEmployee Assistance Program/liliProfessional Development Reimbursement/lili401K with Company Matching/lili10 days vacation PTO/year/lili6 days sick PTO/year/lili10 days holiday PTO/year/liliPotential eligibility for the Public Service Loan Forgiveness Program (PSFL) for federally qualified loans/li/ul/div
