Case Manager

Position Summary:The Individual Home Supports Case Manager is responsible for developing, coordinating, and monitoring individualized supports for adults with developmental disabilities who live independently or in community-based settings. This role provides skill training, supportive counseling, case management, and crisis intervention while promoting personal choice, independence, and community inclusion.
