The ideal candidate will assist with care planning, discharge coordination, psychosocial assessments, and connecting residents with appropriate resources and services. This individual should be highly organized, an excellent communicator, and passionate about advocating for the emotional and social well-being of others. Join a team that values empathy, professionalism, and delivering exceptional resident-centered care.ResponsibilitiesPerform psychosocial assessments for residents and their familiesDevelop, implement, and monitor individualized social service plansFacilitate discharge and transition planningCoordinate with interdisciplinary team (nursing, therapy, medical, dietary, etc.)Provide counseling, crisis intervention, advocacy, and resource linkageAssist with admission evaluations (including PASRR, pre-admission screenings, etc.)Maintain regulatory compliance with state and federal regulations regarding social services in long-term careLead or participate in care plan meetings and family conferencesProvide education, support, and referrals for residents and families (bereavement, community services, financial, legal, etc.)Documentation of social work interventions in the resident’s clinical recordRequirements:Bachelor’s degree in social work (BSW) or related human services field; master’s in social work (MSW) preferredCurrent state social work license or eligibility preferredMinimum 1–2 years’ experience in a healthcare, long-term care, or skilled nursing settingKnowledge of Medicare, Medicaid, and long-term care regulationsStrong assessment, counseling, and communication skillsAbility to work as part of a multidisciplinary teamOrganizational skills and ability to manage multiple casesFamiliarity with community resources and strong referral networkJoin a team committed to putting people first—every resident, every day.COMPANY BENEFITSRelavix Health Group is committed to supporting our team both professionally and personally by offering meaningful benefits designed to promote wellness, financial flexibility, career growth, and work-life balance.
