The LPN monitors resident conditions, administers medications and treatments, communicates changes in condition to the appropriate members of the clinical team, and works collaboratively with RNs, CNAs, physicians, and other healthcare professionals.ResponsibilitiesProvide skilled nursing care according to physician orders and individualized care plansMonitor residents and identify changes in conditionAdminister medications and treatments as orderedProvide wound care and other nursing treatments within scope of practiceCommunicate changes in resident condition to the RN, physician, and appropriate members of the care teamRespond promptly to resident needs, call lights, and emergenciesComplete accurate and timely nursing documentationAssist with admissions, transfers, and discharges as neededCollaborate with RNs, CNAs, physicians, and the interdisciplinary teamProvide compassionate care while promoting resident dignity, safety, and comfortFollow infection prevention and infection control protocolsParticipate in Quality Assurance and Performance Improvement (QAPI) initiativesMaintain compliance with CMS, state, and facility regulationsSupport a positive, respectful, and team-oriented work environmentQualificationsCurrent Missouri Licensed Practical Nurse (LPN) license in good standingCurrent BLS/CPR CertificationSkilled nursing, rehabilitation, long-term care, hospital, or post-acute care experience preferredStrong clinical and organizational skillsExcellent communication and teamwork abilitiesCompassionate, dependable, and resident-focusedAbility to work effectively as part of a day shift nursing teamWe Welcome Nurses With Experience InSkilled NursingLong-Term CareRehabilitationPost-Acute CareHospital NursingMedical-Surgical NursingGeriatric NursingCharge NurseWhy Heritage Wellness & Rehabilitation?At Heritage Wellness & Rehabilitation, you'll be part of a team that values clinical excellence, teamwork, and compassionate resident care.
