You’ll be a part of a supportive multidisciplinary team comprising fellow physical therapists, occupational therapists, and rehabilitation specialists, all working toward a common goal of enhanced patient outcomes. Key Responsibilities Conduct thorough patient evaluations to assess physical function and mobility issues, closely analyzing each individual’s condition. Develop and implement customized treatment plans that address specific orthopedic issues, utilizing techniques such as manual therapy, therapeutic exercises, and functional training. Monitor patient progress throughout treatment sessions, adjusting plans as necessary to ensure optimal recovery and rehabilitation. Educate patients and their families about rehabilitation exercises, strategies for injury prevention, and the importance of adherence to home care regimens. Document all patient interactions and clinical assessments in accordance with organizational protocols and regulatory requirements. Collaborate with referral sources, physicians, and other healthcare providers to coordinate comprehensive care and ensure seamless patient transitions. Patient Population / Client Focus This role specifically serves patients recovering from orthopedic surgeries, sports injuries, and chronic pain conditions. Our patient population often includes individuals seeking to regain functional mobility and independence following major procedures such as joint replacements or repairs. The therapist will work primarily with adults, tailoring therapeutic approaches to meet the unique needs of each patient, encouraging their active involvement in their own recovery process. Qualifications Must hold a valid Physical Therapist license in the state of New York. Completion of a Bachelor's Degree in Physical Therapy is mandatory. A strong foundation in orthopedic rehabilitation principles and a passion for improving patient health outcomes. Preferred Experience Minimum of 2 years of professional experience as a Physical Therapist, particularly in an outpatient orthopedic setting. Experience with home care practices and remote patient management is highly desirable. Familiarity with electronic medical records (EMR) systems is preferred to streamline documentation and patient tracking.
