Medical Social Worker

Lehigh Valley Health Network seeks a compassionate Medical Social Worker to support patients and families across the care continuum. In this hospital-based role, you’ll complete psychosocial assessments, provide counseling and crisis intervention, and coordinate discharge plans and community resources. Working within LVHN’s collaborative culture, you will partner with interdisciplinary teams to address social determinants of health, reduce barriers to care, and promote patient well-being in eastern Pennsylvania.
