Physical Therapist

Role: Licensed Physical Therapist providing personalized in-home, senior living, and virtual care to older adults. Responsibilities Deliver one-on-one physical therapy in private residences and senior communities. Perform evaluations, develop individualized care plans, and administer evidence-based treatments. Support patient wellness, mobility, independence, and aging in place. Document sessions accurately within electronic medical records. Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety and functional activities. Incorporate telehealth and virtual care to enhance access and flexibility. Qualifications & Skills Valid Physical Therapy license or eligibility in practice state(s). Degree from an accredited PT program. Proficiency in computer use and EMR systems. Current CPR certification. Physical Requirements Ability to exert moderate effort, assist with patient transfers, reposition patients, stand/walk for extended periods, climb stairs, and drive between locations, with adherence to PPE protocols.
