Shift Type: 40 hours per week, day shifts, rotating weekends (2-3 per month), on-call required (6 shifts per month including weekends)

Roles & Responsibilities

• Coordinate patient care across interdisciplinary teams to ensure optimal outcomes
• Serve as a liaison between patients, families, physicians, and healthcare beneficiaries
• Develop, implement, and revise case management plans for complex patients
• Monitor resource utilization and appropriateness of care to optimize patient outcomes
• Manage frequent emergency department visitors with focus studies to improve care coordination

Qualifications

• At least 1 year of experience in a Medicare-certified home health agency (required)
• Valid driver's license and personal car insurance (required)
• Current Hawaii state license (required)
• Proficiency with Homecare Home Base and OASIS (required)
• COVID-19 and Flu vaccination (required, no exceptions)
• Bachelor's degree in nursing or related field (preferred)

• Homecare Home Base software expertise
• OASIS assessment skills
• Case management principles and best practices
• Patient care coordination across settings
