Care Lync
Care Lync is seeking a Service Coordinator / Support Coordinator / Case Manager to develop individualized care plans, coordinate services, and monitor outcomes for adults with IDD/AD/Autism in Pennsylvania. You will broker services, monitor ISP implementation, and collaborate with family caregivers to meet housing, medical, social, and vocational needs.
