SUMMARY
Reports to Director Brown University Health Pharmacy, Director Specialty Pharmacy, or Ambulatory Manager as applicable. Under the supervision of the Pharmacist‑In‑Charge, Pharmacy Supervisor, Senior Clinical Pharmacist Specialist, or Clinical Coordinator, the pharmacist is responsible for the interpretation, evaluation, preparation, and dispensing of medications pursuant to legitimate orders or prescriptions in keeping with established policies, procedures, standards of practice, and scope of pharmacy ambulatory services. The pharmacist collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost‑effective medication use. The pharmacist oversees and closely reviews the work of pharmacy technicians, pharmacy interns, and ancillary support personnel. Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served. Ensures pharmacy services are provided in accordance with established policies, procedures, and federal and state rules. Receives patient medication orders or prescriptions from practitioners in the outpatient setting and reviews each order to determine therapeutic appropriateness relative to the patient’s clinical status, concomitant drug therapy, and diet, while considering developmental stages and aging processes. Contacts the prescriber or covering practitioner to initiate interventions that maximize therapeutic efficacy, minimize potential drug therapy risks, and optimize cost effectiveness. Provides optimal patient care by removing barriers to medication access, facilitates patient recruitment, educates patients and families on medication use, delivers accurate information on drug interactions and side effects, and participates in continuous quality assessment and improvement activities. Maintains professional competence through ongoing study, professional activities, and affiliations. Performs immunizations or medication administrations per Rhode Island Board of Pharmacy rules, depending on position duties, and performs other assigned duties. Retail and Specialty Pharmacy Responsibilities Reviews, approves, and enters medication orders into the computerized pharmacy information system with correct formulation, dosage, frequency, and duration. Ensures patient specific parameters (diagnosis, allergies, weight, age, etc.) are documented. Reviews and approves all medication orders and patient specific parameter entries if delegated to technician staff. Dispenses ordered medications ensuring correct quantity, dosage, form, and timing. Approves formulations for compounding or manipulation prior to dispensing. Checks final product prior to dispensing when preparation is delegated to technician staff. Maintains appropriate documentation and retains records regarding drug purchasing, storage, preparation, and distribution in accordance with established policies and federal/state regulations. Additional Specialty Pharmacy Responsibilities Adheres to the policies and procedures of the Specialty Drug Patient Management Program (SDPMP) and encourages patient participation. Performs patient outreach and assessments, participates in the development and execution of patient plans of care. Medication Access Program (MAP) Responsibilities Provides MAP refill authorization services per collaborative practice agreements and protocols. Navigates the electronic health record to investigate and answer questions related to MAP services. Completes and documents medication benefit investigations, prior authorizations, medication reconciliations, and/or medication financial assistance services. Provides alternate therapeutic drug recommendations per established guidelines. Monitors accuracy and efficiency of MAP technician refill processing, prior authorization submissions, denials, appeals, and coordination of medication deliveries. Provides real‑time feedback to improve case management and outcomes, and identifies operational trends to enhance system workflows and MAP success. Ensures MAP activities meet or exceed patient quality standards. Clinic‑Based Program Responsibilities Monitors and interprets laboratory results and adjusts medication regimens per existing protocol. Identifies out‑of‑scope patient care issues and triages them to the appropriate member of the patient’s healthcare team.MINIMUM QUALIFICATIONS
Basic Knowledge Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy. Must possess current unrestricted licensure as a licensed pharmacist or licensed 90‑day temporary pharmacist in Rhode Island prior to start date. Must obtain full licensure within six months of start date and maintain current unrestricted licensure thereafter. For positions performing immunizations per Rhode Island Board of Pharmacy rules, must possess and maintain an ACPE‑approved Immunization Certification and AHA Basic Life Support (BLS) Certification within six months of start date. Experience Two to three years progressively responsible related experience, preferably in a similarly diverse and operationally complex healthcare environment. Knowledge of pharmacy information systems, EHRs, automated dispensing technology, and personal computers. Occasional travel between facilities or sites as required. Effective interpersonal and communication skills. Work Environment and Physical Requirements Extended periods of standing and walking. Requires visual acuity and manual dexterity to operate a computer. Independent Action Performs independently within department policies and practices. Refers specific complex problems to supervisor when clarification of departmental policies and procedures may be required. Supervisory Responsibility None.
