Occupational Therapist (OTR/L) – Full-Time or PRN | Home Health Community-Based | Philadelphia & Bucks County Company: Deer Meadows Home Health Job Type: Full-Time or PRN (Per Diem) Setting: Home Health, Assisted Living (ALF), Independent Living (IL), Personal Care Coverage Area: Northwest Philadelphia, Northeast Philadelphia, Lower Bucks County Job Overview Deer Meadows Home Health is seeking a dedicated Occupational Therapist (OTR/L) for a Full-Time or PRN opportunity in Home Health and Community-Based settings, including Assisted Living (ALF), Independent Living (IL), and Personal Care. This role offers the opportunity to provide one‑on‑one patient care across a diverse population while working with a collaborative interdisciplinary team focused on improving patient independence and quality of life. Key Responsibilities Evaluate patients and develop individualized occupational therapy treatment plans Provide skilled interventions to improve ADLs, safety, and functional mobility Deliver therapy services in homes and community-based settings (ALF, IL, Personal Care) Educate patients and caregivers on adaptive equipment and home safety Collaborate with nurses, therapists, and interdisciplinary team members Complete timely documentation in Home Care Home Base (HCHB EMR) Independently manage caseload and daily schedule Qualifications Graduate of an accredited Occupational Therapy program Active Pennsylvania OT license (OTR/L) or eligibility Home Health experience preferred Experience with Home Care Home Base (HCHB) a plus Strong clinical judgment, communication, and organizational skills Valid driver’s license and reliable transportation Compensation & Benefits Competitive compensation per visit Flexible Full-Time or PRN scheduling Supportive and collaborative team environment Work‑life balance Diverse patient population across multiple care settings Equal Opportunity Employer Deer Meadows Home Health is an Equal Opportunity Employer. #J-18808-Ljbffr Deer-Meadows
