Remote Bilingual Care Manager (CCM/RPM) - Spanish

Pharmko, Inc.

Preventel Health is seeking a bilingual remote care manager to deliver CCM, RPM, and BHI services by telephoning assigned patients. You will collaborate with patients, families, and providers to develop care plans and monitor progress, coaching on chronic condition management and preventive care.

Responsibilities include enrollment facilitation, documentation, and coordination with primary care teams across English and Spanish-speaking patients. Remote work with flexible scheduling is available.
