- Conduct evaluations for patients in both inpatient and outpatient settings, creating tailored treatment strategies.
- Implement therapeutic interventions aimed at enhancing mobility, strength, equilibrium, and overall functionality.
- Collaborate with a multi-disciplinary team, including nurses, physicians, case managers, and rehabilitation professionals, to ensure optimal patient outcomes.
- Provide education for patients and their families regarding therapeutic objectives, discharge plans, and home exercise protocols.
- Maintain precise documentation of patient evaluations, treatment plans, and progress, adhering to the necessary guidelines.
- Engage in care team discussions and interdisciplinary meetings for seamless patient care coordination.
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program.
- State licensure as a Physical Therapist .
- CPR certification is preferred.
- Additional specialized certifications (e.g., in neurology, geriatrics) are advantageous but not mandatory.
- Prior experience in a clinical setting is highly preferred.
- Ability to deliver care to patients with complex medical conditions and mobility challenges.
- Strong interpersonal and collaborative skills.
