Description
  • Conduct evaluations for patients in both inpatient and outpatient settings, creating tailored treatment strategies.
  • Implement therapeutic interventions aimed at enhancing mobility, strength, equilibrium, and overall functionality.
  • Collaborate with a multi-disciplinary team, including nurses, physicians, case managers, and rehabilitation professionals, to ensure optimal patient outcomes.
  • Provide education for patients and their families regarding therapeutic objectives, discharge plans, and home exercise protocols.
  • Maintain precise documentation of patient evaluations, treatment plans, and progress, adhering to the necessary guidelines.
  • Engage in care team discussions and interdisciplinary meetings for seamless patient care coordination.
Education
  • Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program.
Certifications
  • State licensure as a Physical Therapist .
  • CPR certification is preferred.
  • Additional specialized certifications (e.g., in neurology, geriatrics) are advantageous but not mandatory.
Skills
  • Prior experience in a clinical setting is highly preferred.
