Hospice Field Staff RN/Case Manager

Hospice of Southern Illinois, a not‑for‑profit hospice, is hiring a Hospice Field Staff RN/Case Manager to work at the Belleville office and visit patients throughout the Southern Illinois and St. Louis Metro‑East region. Position Summary Staff RN evaluates patients, families, and caregivers; plans and implements care plans; participates in the inter‑disciplinary team. Manages patient needs and care coordination from admission to discharge. Compensation & Work Schedule Pay : $39.21 per hour, full‑time non‑exempt, 40 hours per week. Schedule : Monday‑Friday 8:30 AM‑5:00 PM with rotational on‑call evenings, weekends, and holidays. Location : Belleville Patient Services Office; travel to patient homes, long‑term care, nursing homes, hospitals, etc., requiring a valid driver’s license, insurance and reliable transportation (reimbursement $0.70 per mile). Key Responsibilities Maintain a caseload of patients and coordinate care with Hospice Aides and the IDT. Provide direct patient care, including medication orders, pain and symptom assessment, and physical examinations. Communicate findings to the hospice physician and obtain medication orders. Update and review the Plan of Care on each visit and supervise Hospice Aide visits every 14 days. Educate patients, families, and caregivers on end‑of‑life care and pain management. Work on‑call when the regular nurse is unavailable. Qualifications & Requirements Registered Nurse with current Illinois RN license. Experience in hospice or palliative care is preferred. Strong clinical skills and demonstrated ability to work independently. Excellent communication skills and ability to work collaboratively in a multidisciplinary team.
