Short Description:

The Transition Specialist for Indiana's Department of Correction: Division of Youth Services provides oversight to all transition-related practices in the State's juvenile correctional facilities.

Complete Description:

Job responsibilities may include, but are not limited to: facilitating the implementation of a life skills curriculum, titled Next Steps. Utilizing the Transition Coordinator Handbook in an effort to standardize the transition practices. Evaluating the five "Transition Consultations" to be completed by every student under the guidance of an assigned teacher. Participating on a weekly basis in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans. Doing follow-up phone calls to released youth at 1, 3, 6, 9, and 12 months. Representing the Indiana Department of Correction- Division of Youth Services at many conferences and meetings. Assisting in the development of a partnership with Indiana's Department of Workforce Development, HIRE, and Indiana University's HOPE Mentoring. Assisting in the development of the individualized Education Learning Plan for each youth upon entry at the facility. Working with state, county, and private agencies to provide re-entry services for youth transitioning back into the community. Assisting students with obtaining social security cards, state identification, driver's license, birth certificates, and additional documentation. Assisting students with transitioning back to public schools. Assisting students with employment, apprenticeship programs, and military service information.
