We are seeking a Physical Therapist (PT) Skilled Nursing Facility (SNF) for a 13-week contract in Hayward, California. This position provides physical therapy services in a skilled nursing setting with 30 guaranteed hours per week.

Candidates must be available to work either a Sunday Thursday or Tuesday Saturday schedule.

• Provide physical therapy services within a Skilled Nursing Facility (SNF) setting.

• Deliver rehabilitation services to assigned skilled nursing patients.

• Support patients in achieving functional rehabilitation goals.

• Maintain appropriate physical therapy documentation for assigned patients.

• Collaborate with the rehabilitation team to support continuity of patient care.

• Current, Active Physical Therapist License Required; Specific State Licensure Requirement Must Be Verified.

• Less Than 1 Year of Physical Therapist Experience Accepted Per Requisition.

• Ability to Work in a Skilled Nursing Facility (SNF) Setting Required.

• Availability for a Sunday Thursday or Tuesday Saturday Schedule Required.

• Ability to Meet a 30-Hour-Per-Week Guaranteed Schedule Required.
