Seven Healthcare are seeking a dedicated Physical Therapist for a contract assignment in Cedartown, GA. This travel position involves working within Bartow and Paulding Counties, providing essential therapy services to patients based on physician orders/referrals. - Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnosis, medical history, precautions, and contraindications
- Establish measurable goals and develop a treatment plan for maximum independence
- Regularly re-assess treatment effectiveness and adjust goals or treatments as needed
- Communicate patient needs and progress to treatment team, physician, and family
- Consult with medical providers regarding evaluations, treatment progress, and discharge
- Recommend and assist in securing adaptive equipment for patients
- Maintain and submit documentation as required by regulations
- Supervise Physical Therapist Assistants and Aides in patient care activities
- Participate in staff development programs and continue education as required
- Valid PT License
- Experience with HCHB and OASIS
