Per Diem Residential Counselor- HARP

BHcare is seeking a compassionate and dependable Transitional Counselor to support individuals in developing independence and improving their quality of life. In this role, you will provide emotional support, counseling, and skill-building in areas such as daily living, health and safety, financial management, and community integration. Transitional Counselors play a key role in creating a respectful, supportive environment that promotes recovery and growth.

This position is a part-time position, 16 hours per week. The schedule is Saturday and Sunday from 11pm-7am. Key Responsibilities: Assist residents with independent living skills, including meal planning, cleaning, hygiene, and money management. Support clients in developing and following recovery plans, coordinating with treatment teams and natural supports. Provide advocacy, referrals, and connections to community resources. Facilitate rehab groups and interventions focused on skill building and recovery. Supervise self-medication protocols and ensure accurate documentation. Provide crisis intervention and collaborate with emergency services as needed. Maintain clear communication through progress notes, logs, and required records. Ensure a safe and supportive environment, including overnight coverage when assigned. Provide transportation and assist with scheduling personal appointments.

Qualifications

Minimum Qualifications

• Two years of college education or High School Equivalent and 2 years direct care work experience
• Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
• Must have and maintain a valid driver's license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization's auto insurance plan.

Professional Licensure/Certifications

• Ability to obtain and maintain Basic Life Support (BLS) certification
