This award-winning team is looking for a  Full-Time   Registered Nurses  to manage patients in the following counties:  North Fulton, Forsyth, Dawson, Lumpkin & Gainesville.  The ideal candidates want to join a phenomenal hospice team, enjoy working in home care and have a passion for end of life care.

QUALIFICATIONS:

• Currently licensed as a Registered Nurse in Georgia.
• One year of hospice or home health experience, preferable.
• Minimum 2 year’s experience as a Registered Nurse.
• Reliable transportation with appropriate insurance coverage
• Good communication skills (verbal and written)

REGISTERED NURSE RESPONSIBILITIES:

• Regularly assesses and reassesses the nursing needs of the Hospice patient.
• Provides Hospice nursing services, treatments, and preventive procedures.
• Initiates nursing procedures appropriate for the patient’s Hospice care and safety.
• Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition.
• Teaches, supervises, and counsels the Hospice patient and family members about providing care for the patient.
• Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
• Attends team conferences.
• Works with the marketing team to educate the community about hospice services.
