Job Summary:
Seeking a compassionate Occupational Therapist to provide intermittent, home-based therapy services for senior patients. The OT will evaluate patients, develop individualized care plans, implement therapeutic interventions, monitor progress, educate patients/caregivers, and maintain accurate documentation.

Job Summary:
Seeking a compassionate Occupational Therapist to provide intermittent, home-based therapy services for senior patients. Key Responsibilities:
- Provide prescribed OT services to senior patients in their homes
- Conduct evaluations and develop treatment plans
- Implement therapeutic interventions to improve functional skills and independence
- Monitor patient progress and adjust care plans
- Communicate with physicians and care team
- Educate patients and caregivers on exercises/home programs
- Maintain compliant clinical documentation
- Collaborate with interdisciplinary teams
- Supervise therapy support staff as needed
- Graduate of an accredited OT program
- Current Arizona OT license
- Minimum 2 years clinical experience preferred
- Strong interpersonal, organizational, and documentation skills
- Home health care experience
- Proficiency in upper extremity rehab and ADLs
