Hospice Transition Nurse — Education & Care Coordination
St. Croix Hospice
St. Croix Hospice is seeking a Hospice Transition Nurse in Huntley, IL to serve as a resource for hospital patients, families, and staff. You will manage admissions, GIP patients, and help ensure smooth transitions for patients and families, while building referrals from hospitals and nursing homes. The role requires an active RN license, ability to travel between about 5-6 hospitals, and coordination with admission personnel and the Hospice Medical Director. #J-18808-Ljbffr St. Croix Hospice
