Nurse Care Manager

The Nurse Care Manager provides culturally competent, trauma-informed care to consumers through assessment, care coordination, advocacy, and person-centered planning. Under the direction of the Supervisor of Integrated Health Services, this role coordinates medical, behavioral health, and community services to ensure timely, appropriate, and goal-oriented care. The position supports the LifeWays Substance Use Disorder Health Home (SUDHH) model and upholds LifeWays mission, vision, and values.Key ResponsibilitiesEngage consumers with empathy, professionalism, and a non-judgmental approachConduct assessments, reassessments, and person-centered service planningCoordinate referrals and care within and outside the provider network, including Outpatient PsychiatryCollaborate with psychiatrists, primary care providers, specialists, and community partnersMonitor caseloads and ensure ongoing medical necessity and appropriate levels of careSupport transitions across levels of care and life stagesAssist with crisis and after-hours planningProvide services in person, by telehealth, and by phonePerform care management functions for the SUDHH program
