Role Description The Registered Nurse at Apex Community Care, Inc. is a full-time, on-site role based in Danbury, CT. This role provides direct nursing care, including assessment, care planning, medication administration, and monitoring of individuals living with HIV/AIDS, physical and mental health conditions, and substance use disorders. The nurse collaborates with an interdisciplinary team to coordinate care, support treatment adherence, and ensure continuity of services across medical and behavioral health providers. Daily responsibilities include documenting clinical encounters, educating clients and families about health management and prevention, and responding to changes in client conditions. The nurse also participates in quality improvement initiatives and helps maintain a safe, trauma-informed, and culturally responsive care environment. Qualifications Active Registered Nurse (RN) license in the state of Connecticut and completion of an accredited nursing program. Clinical experience in HIV/AIDS care, community health, or related field, or strong interest in these areas. Proficiency in patient assessment, care planning, medication management, and documentation in electronic health records. Strong communication and interpersonal skills for collaborating with multidisciplinary teams and providing client-centered education. Ability to work effectively with diverse populations and maintain a non-judgmental, inclusive, and trauma-informed approach. Solid organizational and time-management skills to handle multiple clients, priorities, and documentation requirements. Basic computer literacy, including familiarity with EHR systems and standard office software. Current BLS/CPR certification; additional relevant certifications (e.g., psychiatric nursing, addiction nursing) are a plus.
