As a Hospice Social Worker (BSW), You Will:

\n • Provide psychosocial support to patients and families in accordance with an individualized plan of care
\n • Conduct assessments to identify social, emotional, financial, and environmental needs
\n • Use professional judgment to guide patients and families through the challenges of illness and loss
\n • Offer counseling, education, and referrals to community resources
\n • Collaborate closely with the hospice interdisciplinary team to support the whole-person needs of those in your care
\n • Monitor changes in patient/family circumstances and adjust services accordingly
\n • Promote dignity, respect, and advocacy for every patient and family you serve

Qualifications – What You’ll Bring:

\n • Bachelor’s Degree in Social Work (BSW) from a CSWE-accredited school
\n • Licensure as required by the state of employment
\n • At least 1 year of experience as a Social Worker in a healthcare or hospice setting
\n • Knowledge of the psychosocial dynamics of illness, grief, and end-of-life care
\n • Compassion and tact when supporting patients and families during emotionally intense experiences
\n • Ability to work independently while contributing meaningfully to a care team

Preferred Qualifications (Not Required):

\n • 3 to 5 years of hospice or healthcare social work experience
\n • Familiarity with terminal illness, grief response, and crisis counseling
\n • Experience working with diverse populations and family systems
\n • Advanced training or certifications in grief counseling or palliative care
